Welcome to Session 3! Please find below some documents to download. These forms are also in the Google Drive.
During Session 3 we will be talking about building routines during play.
You will fill in some of these documents during Session 3, and you will fill some in at home afterwards. It is advised that you to print out your completed home plan to keep it handy during the week when you are trying out your plan.
You should be able to fill in all the forms below electronically by clicking boxes, or selecting an area to type text into. Carolyn will have already notified you of the password at the ‘Meet and Greet’. If you have any trouble please contact Carolyn.
If you are completing the form during a session please (if possible) have a 2nd screen (not a phone) so you can fill it in, or else you may wish to print it out if you can’t complete the forms on a device.
In all cases, please share your completed forms with Carolyn via email by the deadlines below, so that she can offer support during follow up sessions and especially during your video sessions. If you have to print them out, you can take a photo or scan then and return by email / WhatsApp / Text.
Week 3 Home Plan
Please download your Home Plan template here.
You will need to fill out this form during Session 3 (Carolyn can help you do this), and then you will try out the plan at home during the week, every day if you can. The more repetition, the better! Little and often is best. Please send your completed form by email by 5pm the day after session 3.
After Session 3, please complete and return to Carolyn via email to help her tailor the programme to your needs, and improve future programmes. Please return this by 5pm the day after Session 3.

Once you have tried out your home plan throughout the week, please fill in the Report Back form and return it to Carolyn via email by Monday.
